Objective
Following recommendations from the Royal Commission into Aged Care Quality and Safety, this project aimed to improve information sharing across aged care and health systems. Specifically, it focused on integrating aged care client support plans from My Aged Care into My Health Record — enabling healthcare providers to access critical care data when it matters most.
Our Role
We led the end-to-end delivery of this complex, cross-agency digital integration project. Key responsibilities included:
- Enabling My Aged Care to act as a Registered Repository for support plan sharing.
- Implementing secure linkage to Services Australia’s Healthcare Identifier (HI) service.
- Designing consent-based workflows to upload and update client care plans in MHR.
The project addressed long-standing gaps in aged care information accessibility, particularly as individuals transition between acute, primary, and residential care settings.
Our Approach
A structured, collaborative approach underpinned delivery:
- Prince2 Framework: Ensured disciplined governance for a highly legislative project.
- Multi-agency Delivery: Coordinated across DoHAC, ADHA, and Services Australia via a unified project plan.
- Agile Tools: Used Azure DevOps for requirements tracking and GovTeams for real-time collaboration.
- Strong Governance: Weekly and monthly cross-agency meetings, shared RAIDD logs, and transparent reporting supported progress and accountability.
Outcomes & Impact
- Care plans are now accessible in My Health Record with client consent
- Reduced duplication of records and testing across aged and healthcare systems
- Improved data accuracy through integration of Healthcare Identifiers
- Full project delivery within 11 months, despite previous delays of over two years
- Delivered on time and within an $18 million budget
